Faculty Alternate Assignment – Dynamic Forms Instructions for Faculty  

 

The digital version of the Faculty Alternate Assignment form is constructed with built-in routing and messaging. This form is designed for the administrative assistant in each academic division to initiate and route to the appropriate people for additional information, approval, and completion.   

If you received an email notification from notify@ngwebsolutions.com of a pending Faculty Alternate Assignment, please follow the procedures below to access and complete the form. If you have questions at any time during this process, please email the administrative assistant in your division.   

You must be signed into the MyMassasoit Portal to access your form information. You may see a window asking for your Microsoft account information if you are not already signed into the portal. If you are a first-time user or are new to Dynamic Forms, detailed instructions on accessing the platform are located in the Knowledge Base. 

  1. Once you have supplied your Massasoit credentials you will be routed to Dynamic Forms and presented with the Faculty Alternate Assignment form. The administrative assistant has completed the prepopulated information. Please review this information.   

*If you have been assigned a Project as your Alternate Assignment, be sure to supply the description of your assignment and attach your documentation in the (Faculty Member Completes) section.  This section only appears on the form if you are required to complete it. 

You have the option to approve your form by signing, or you can return your form to the administrative assistant for revisions. If there are corrections to be made on the form by the admin see Section 7 below for instructions.

  1. To sign and approve your form, click in the highlighted area to apply your signature.  

  

  

  1. Be sure to submit your form when routed back to review your signature.   

   

  1. A pop-up message will appear noting that your form has been submitted along with a link to view your PDF.   

                                    

  

  1. You will then receive an email notification message that your form has been routed to your dean for review. A message will also be sent to the administrative assistant who initiated the form.    

           Please add notify@ngwebsolutions.com to your safe senders' list to ensure notification emails do not land in your junk or spam  

           folder.   

  

  1. If there are errors on the form, you may return the form for revision to the administrative assistant and begin the signing process again. Click on the “Return for Revision” button on the bottom of the form. When the email opens, please note your name and Missed Class Form Return for Revision in the subject line along with the reason for the return in the body of the email.   

  

   

   

  

  1. The administrative assistant will receive your email request to update the form.  

  

   

  1. After the administrative assistant updates and resubmits the form, you will receive the email notification with a link to update the form and begin the signing process again.    

  

Note: The dean and vice provost have the option of returning the form to any previous signer for revision (choice of one signer in dropdown). An email notification message from notify@ngwebsolutions.com  with a link to the form will be sent to the person selected. The participants preceding the signer will all receive a notification that the form was returned for revisions.  

  

  1.  Once your form has been approved by the dean and vice provost, you will receive an email message like the one below.   

            

   

You may return to Dynamic Forms to review forms that you have signed at any time by clicking on the My Forms link at the top right of the screen and selecting Forms History.
