Missed Class Form (DCE only)

Missed Class Form (DCE only) - Form Instructions   

The digital version of the Missed Class Form is constructed with built-in routing and messaging. This form is designed to be routed for approval and completion.   

Always be sure your MyMassasoit Portal is open when creating or signing Dynamic Forms from links received in email or from documentation. If you have not already signed into the portal, you may see a window asking for your Microsoft account information.   

 To initiate the form, go to ITS Help Desk – Services – Administrative Services 

Step 1   

  1. Select the administrative assistant and dean you would like to route your completed form to by clicking on the prospective drop-downs. Your form will populate after you click the Continue button.

  1. Your name will be pre-filled in on the form. Fill in the additional required information noted by a red asterisk.

 3. Check the box which applies and complete the information noted for that box. Be sure to upload your files if options 3 or 4  are chosen. Multiple files can now be uploaded when selecting the 'Choose File' button.

  1. Once your form is complete, click on the highlighted area to sign your form.  Make sure you click on the Submit Form button after electronically signing to route the form to the administrative assistant for your division.

     

  

  1. Once submitted, a confirmation message with a link to view your PDF  will come up to let you know the form has been submitted and routed to the administrative assistant who will review and record your submission and then route to your dean.

    Step 2 

  1. Upon submission should additional information be required; you will be contacted directly by the Administrative Assistant or Dean. 

  1. Once a decision has been reached you will receive an email from Dynamic Forms indicating approval or denial of the request.
